1. Nature of Services

   • Enrichment classes and camps for TK - 8th grade students, held on school campuses.

2. Enrollment and Program Structure

   • Class groupings and durations are subject to change based on program needs.

3. Fees, Payments, and Billing

   • Payment is required upfront via our online portal or through school/district arrangements.

   • Various payment methods are accepted, subject to validation checks and authorization by the card issuer.

   • The Company reserves the right to revise service prices and will notify of any changes.

4. Liability Release and Assumption of Risk

   • Participants and guardians acknowledge and accept the risks inherent in physical activities associated with the program.

   • Level UP Enrichment is released from liability for any loss, damage, injury, or death that may occur during participation.

5. Student Code of Conduct and Disciplinary Action

   • Adherence to the Code of Conduct is required.

   • Disciplinary actions for violations are outlined in the program's disciplinary policy.

6. Cancellation, Refund, and Order Policies

   • Specific conditions for enrollment cancellations and service fee refunds are stated, including pro-rated refunds within the first two weeks.

   • The Company reserves the right to refuse or cancel enrollment under certain conditions.

7. Safety, Liability, and Medical Consent

   • Safety protocols must be adhered to, with medical liability release and emergency care consent required from guardians.

14. Enrichment classes and camps require that students be able to participate independently. Instructors and vendors are not District Employees and are not required to provide accommodations under IEP or 504 plans. During registration please provide information concerning special needs that your student may have to help instructors provide a safe and successful environment, such as medical conditions, allergies, and instructional accommodations.
